What is Bypass Surgery (CABG)?

Coronary artery disease is commonly characterized by the deposition of fatty substances in the heart walls, narrowing the coronary arteries, and restricting the blood supply to the heart. Coronary bypass grafting surgery is performed to divert blood flow away from a section of the blocked arteries in the heart. Heart bypass surgery does not cure heart disease but is rather performed to mitigate the symptoms caused by myocardial ischaemia, such as chest pain and breathlessness. Coronary artery bypass grafting surgery reduces the risk of mortality due to a heart attack.

Why is CABG performed?

Less supply of oxygen leads to dysfunction or dysregulation and organ damage; therefore, to treat these narrowed blood vessels, medication and stent therapies are employed initially, but in severe cases where there are multiple blocks in the heart,  CABG surgery can serve as a better surgical option, as it involves grafting a healthy blood vessel from the leg or femoral artery to bypass the blocked arteries and restore the normal blood flow. The heart bypass surgery may be performed as an emergency treatment for a heart attack.

In contrast with other replacement and repair surgeries, coronary artery bypass grafting (CABG) is commonly used to significantly enhance the quality of life for patients with complicated cardiac disorders. This combined strategy effectively treats damage to vital heart valves (the aortic and mitral valves) and blocked arteries.

Types of Coronary Artery Bypass Grafting Surgery:

  • On-pump CABG surgery: For many CABG procedures, temporarily stopping the patient’s heart is required. A cardiopulmonary bypass machine, or heart-lung machine, is connected to maintain breathing and blood circulation throughout the surgery. After the procedure, the machine is detached, allowing the heart to resume beating on its own.
  • Off-pump CABG Surgery: Off-pump heart surgery is an alternative to on-pump procedures, where the heart is stopped and a cardiopulmonary bypass machine is used while the patient is on the ventilator, and is specifically used for heart bypass surgery or coronary artery bypass surgery.
  • Minimally Invasive CABG: This procedure thoroughly assesses the heart through smaller incisions rather than traditionally opening the breastbone. This approach can be performed on-pump and off-pump, depending on the patient’s needs and the surgeon’s preference, offering faster heart bypass surgery recovery times and less pain.

Who Needs Coronary Artery Bypass Grafting Surgery?

  • Individuals with improper blood flow due to blocked arteries in the heart.
  • Effective in treating individuals with a damaged heart valve.
  • For those with existing heart problems from birth, a condition called a congenital heart defect.
  • Individuals who suffer from severe heart problems, such as heart tumors, irregular heartbeats, severe heart failure, and weakening or bulging blood vessels that may eventually burst.
  • More damaged arteries and the left heart chamber don’t function.
  • A blocked heart artery that cannot be treated with coronary angioplasty, either with or without stent placement.

CABG Procedure Details

What Happens Before the Surgery?

The surgeon will give you a detailed rundown of the surgery prior to the actual bypass surgery. After taking consent, your medical history will be reviewed. You should not eat for at least 8 hours before the surgery. The hospital may perform a pre-operative screening test.

General anaesthesia will be administered before starting the bypass surgery. After securing the intravenous access and putting in a catheter to drain the bladder, an incision is made in the leg or arm to remove a small portion of the vein to prepare a graft. Next, an incision is made on the chest to place the graft.

Steps Involved in During the CABG Surgery:

Making the Incision and Gaining Access: The surgeon makes a long incision on the chest through the center of the breastbone and splits the ribcage to access the heart.

Heart-Lung Bypass (On-Pump CABG): After reaching this phase, the surgeon may or may not use a heart-lung bypass machine (off-pump CABG or on-pump CABG) to take over the heart’s pumping function, thus allowing the heart to be temporarily stopped.

Harvesting the Graft(s): Firstly, the surgeon confirms the suitability of the veins from the chest (internal thoracic vein) or the leg (saphenous) and then removes and prepares them for the grafts.

Creating the Bypass: Subsequently, one end of the graft is attached to the coronary artery above the blockage, and the other end is attached just below the blockage and sewn to ensure proper blood flow, thus creating a new pathway for blood flow.

Restoring Heart Function: Once the latter is completed, normal functions are triggered using electrical stimulation, and the heart-lung bypass machine is turned off if attached.

Closure: The breastbone is either clamped or wired back in, and the incision is sealed with sutures.

Post-operative Phase of Coronary Artery Bypass Grafting (CABG):

Initial Recovery Phase:

  • Patients are required to stay in the hospital for 2-3 days right after the surgery for the initial heart bypass surgery recovery time.
  • The surgeon monitors the heart and blood pressure and regulates the breathing.
  • The appliances are removed gradually as the patient improves.
  • Subsequently, the breathing tubes are taken out of the patient’s mouth, and they regain the ability to breathe independently.
  • Once the patient can tolerate eating orally, the IV and feeding tubes are removed.
  • Finally, a chest tube is inserted to drain the fluid and is usually removed within a few days.

Mid Recovery Phase:

  • This phase usually occurs under the cardiac rehabilitation program, where the goal is to strengthen the heart muscles and enhance cardiovascular functions for 4-6 weeks after the surgery.
  • Light walking promotes circulation but should be done within comfort limits.
  • The surgeons encourage the patients to resume their physical activity gradually and increase the ones they are already performing.
  • Avoid heavy lifting and strenuous activities for several weeks.
  • Smoking and alcohol should be restricted as they interfere with the healing process.
  • Watch for signs of complications, such as fatigue, mood swings, loss of appetite, and difficulty sleeping.
  • The wound care instructions, along with compression dressing, are given by the surgeons to compensate for the swelling.

Final Recovery Phase:

  • Patients, after three months of recovery, experience improved stamina, reduced chest pain, improved body energy levels, and fewer mood swings.
  • Focus on a healthy diet that includes fruits, whole grains, vegetables, and a low-sodium diet, and limit the consumption of fatty foods.
  • Take the prescriptions as prescribed by the surgeons.
  • Attend follow-up appointments.

Frequently Asked Questions

Heart Bypass Surgery is suggested for those who experience severe chest pain caused by narrowing blood vessels supplying the heart. As the blood supply to the heart decreases, even light exercise can increase the severity of the symptoms. Because Heart Bypass Surgery is an open surgery, it is usually performed when other methods of reopening the blockage, such as angioplasty, fail. Heart Bypass Surgery can also be an emergency procedure during a heart attack.

Like any other surgery, CABG also carries a set of risks and complications that are minor, measurable, and treatable, such as wound infections, irregular heartbeats, and, rarely, a heart attack.

When severe blockages in the coronary arteries significantly block the blood supply to the heart musculature, leading to angina and the risk of heart attacks, despite alternative medical treatments.

The optimal choice between the two depends on the patient’s overall condition, location, number of blockages, and more. While CABG is performed for multiple blocked arteries, angioplasty is performed for a single artery or less severe blockages.

It usually takes 12 weeks to recover completely from Heart Bypass Surgery. However, if you suffer from any complications during or after the surgery, the recovery may take longer than 12 weeks. Therefore, regular follow-up visits should be done to assess the recovery rate.

Yes! CABG, or heart bypass surgery, is a type of open-heart surgery in which the patient’s heart is opened and replaced with a heart-lung machine that pumps blood throughout the body. However, with the traditional approach and the development of the surgery over time, CABG can now be performed minimally invasively, involving smaller incisions and removal of the heart-lung machine.

The person’s life expectancy after undergoing Heart Bypass Surgery can be equivalent to that of an average person. However, Heart Bypass Surgery does not treat the underlying condition. Hence, the disease can come back again if preventive measures are not taken. Therefore, after undergoing Heart Bypass Surgery, treatment of heart disease should be given importance as well.

The exact duration of the surgery varies based on several factors, such as the number of blockages, the severity of the condition, the surgery type, and whether a heart-lung machine is involved. The surgery typically takes between 3 and 6 hours to complete.

You may experience brief episodes of sharp pain in the chest after undergoing Heart Bypass Surgery. In addition, the site of the incision on the chest and the area from which the vein was taken may become site and swollen. However, you will get relief from these symptoms within 4-6 weeks.

Factors such as improvement in lifestyle choices, medications, angioplasty and stenting, a minimally invasive approach, and other non-invasive therapies help delay or avoid the bypass surgery.

Yes, you can lead a normal life after undergoing Heart Bypass Surgery. The prognosis following Heart Bypass Surgery is usually good. Your condition will improve and your life expectancy can be compared to a healthy person. Moreover, you won’t experience symptoms such as chest pain and breathlessness after undergoing Heart Bypass Surgery.
